Medical Clinic Partnerships

We are currently delivering Gerontology Sessions through unique, collaborative and innovative partnerships with local family medical practices utilizing a Social Prescribing framework. Social Prescribing is a holistic approach to healthcare that bridges the gap between clinical care and social care that provides a way for healthcare providers (doctors, nurse practitioners and other healthcare providers) to formally refer patients to non-clinical programs and services.

We are currently offering Gerontology & Aging Navigation sessions at the following Richmond locations:

These unique partnerships allow us to work alongside Physicians, Nurses, Nurse Practitioners and other health practitioners in the practice to best meet the needs of their aging patients and their families. A process is also created to allow any team members to easily refer patients to us, share pertinent information and receive follow-up and status updates about their patients.

​

This approach provides busy medical professionals, who are often ill-equipped, unprepared and unsupported to address their aging clients’ non-medical issues and challenges, with an easy solution for their patients to get the appropriate help and support they need. This will not only reduce  “non-medical” visits to Physicians to address aging issues, save time and resources, but also, widen the lens that is used to address the needs of older adults and those supporting them in a more holistic way.
